This is an addon Shattered Skyrim, the mod that turns Skyrim into a ruined province.
This mod adds in standing stones outside of Angi's cabin where traits with buffs and debuffs can be found. Currently there are 8 traits, all of these perks can drastically change how a character is played. You can have as many traits as you like, only one trait is mutually exclusive with another.

The traits and their effects:
Small - Gain +10 to sneak but lose 15 HP
Large - Gain 15 HP but lose 10 sneak
Brawler - Gain +5 to unarmed damage but lose 5 points in every physical combat skill
Experienced - 10% experience buff but you lose 5 points in every skill
Undead scourge - 10% melee damage reduction to all undead but you take 25% more damage from dragon's melee attacks
Lifeforce - You lose 25 HP but gain 25% magicka regen
Anti-social - You deal 20% more damage to all wildlife but you deal 15% less damage to all humanoids and lose 10 points in Speech
Glass Cannon - You deal 15% more damage and gain a boost of 5 to all combat skills but you take 25% more damage from enemies.
