This is just a totally empty "missing icon" "icon", rather than the Pip-OS Icon Not Found icon.
CPI provides a farting vault boy for this icon, Vanilla HD Missing Icon provides and HD version, but I'm fine with there just literally being no icon present for some things. When using B42 Notify I see the missing icon icon most commonly since most items in the game don't have a small icon listed on their entry, and I'd prefer there to simply be no icon when there isn't one listed for an item.
